Below is contact info and reference to survey instrument used by colleges and universities to assess incoming first year students (usually 17-19) and used in limited way with younger teenagers. What you would need to do is figure out the relationship between the survey instrument and systems thinking. This would require learning more details about the Perry Scheme and mapping it to systems thinking skills and capabilities.
This is one possible approach.
